What to have ready
The form asks about your AMTA coursework, your practicum placements, the instruments you play and your approach to music therapy. Four documents and one video are submitted with it, so it is worth gathering them before you start.
- A letter of eligibility from your AMTA-approved university music therapy program director.
- A letter of recommendation from someone who knows you well and has worked with you in a professional setting, witnessing your musical, clinical and professional skills.
- An up-to-date resume detailing your academic, professional and relevant personal information.
- Your university transcript or transcripts — up to four files.
- A link to a video of yourself answering five questions about your interest in hospice music therapy, and playing three hospice-appropriate songs plus one of your own choosing.
Uploads are accepted as PDF or DOC files, up to 10 MB each. The letter of eligibility and the letter of recommendation may be emailed directly to jbrown1@trustbridge.com if that is easier.
